Volunteer Forms

Volunteer Guidelines:
This form provides detailed information about the Center for Civic Engagement & Service’s definition of volunteer hours and service hours. We have included examples and well as brief information for student organizations interested in the President’s Service Award.

Volunteer & Service-Learning Log:
This document allows students to track their volunteer hours throughout the semester to post them on their transcript. Hours must be submitted, along with a business card or letterhead from the organization signing the form, to the CVC by Reading Day for Fall and Spring semesters and the first business day in August for Summer. If students are not registered on our website upon submitting the form, they will be manually registered by the CVC staff.

Student Club/Organizations Service Log:
This document allows student organizations to keep track of the total volunteer hours performed as a group. It should be submitted the last week of March in the Spring Semester when applying for the President’s Service Award. For more information click HERE!

Volunteer Risk Waiver (FAU Volunteers):
This form must be signed by FAU volunteers who will be participating in any of the activities and events sponsored by the Volunteer Center that are hosted off campus.

Volunteer Risk Waiver (Non-FAU Volunteers):
This form is a risk waiver that must be signed for volunteers outside of the FAU community who are volunteering at FAU or through an opportunity which was presented by FAU.
